Here's what's Included in Your Initial 45 minute Evaluation:

  • Consultation

  • Records review

  • Examination (orthopedic, neurologic and chiropractic)

  • Gather information to prepare a health and wellness, movement improvement plan

  • Chiropractic evaluation

  • Chiropractic adjustment (if appropriate)

  • Discussion of next steps

Note:

Appointments cancelled with at least 24 hours’ notice are eligible for a full refund.

Cancellation & Refund Policy

Cancellations made at least 24 hours in advance of your scheduled appointment are eligible for a full refund. If cancelled within 24 hours, a $25 cancellation fee will be retained.

Once your exam or adjustment has been completed, no refunds will be issued, and any remaining balances from services rendered will be due.

This policy applies to appointments that include:

  • Scheduled appointments

  • Screening appointments

  • Consultation with health history

  • Examination

  • General nutritional evaluation (additional paid testing may be recommended following the consult)
